Beyond the Air We Breathe: The Hidden Menace of Aspergillosis
1. Understanding Aspergillosis
Aspergillus is ubiquitous, its spores float in indoor and outdoor air and are inhaled by virtually everyone, every day, without incident. Healthy lungs will easily take them out. But in susceptible patients, the same spores can cause life-threatening disease. Aspergillosis can cause a spectrum of disease from allergic reactions to chronic lung disease to invasive bloodstream infection. The most frequent species involved, A. fumigatus, A. flavus and A. terreus, differ in behaviour and critically, in antifungal susceptibility. More than 2,113,000 people worldwide are diagnosed with invasive aspergillosis each year and the crude mortality is 85.2%1. In India alone, an estimated 57.25 million people are affected by serious fungal diseases including an estimated 250,900 cases of invasive aspergillosis each year2 .
2. Diagnosis: molecular diagnostics are changing the game
Culture and histopathology were the diagnostic mainstay for decades. Culture can do susceptibility but both have low sensitivity and slow turnaround. A negative culture doesn't rule out disease. Both have value. Faster tests exist (galactomannan and β-D-glucan) but performance varies by population and specimen and neither identifies species involved. Real-time PCR delivers results in hours rather than days, detects lower fungal burden than culture often does. The evidence is mounting for combining molecular testing with biomarkers rather than choosing one. BAL-fluid PCR is highly sensitive.
Detection of Aspergillus DNA alone in a clinical sample, however, is only half the story. The ability to differentiate at the species level directly influences treatment decisions: A. fumigatus is usually treated with voriconazole or isavuconazole (although resistance to azoles is on the rise), A. terreus is intrinsically resistant to amphotericin B, which might be used empirically and A. flavus has its own susceptibility profile3 Without this resolution physicians are left to treat “presumed aspergillosis” in a broad sense, delaying correct treatment.
This is the case for a syndromic panel approach, one test to detect the genus, differentiate key species and include an internal control, all from one limited respiratory specimen. It replaces a number of slower sequential tests with one rapid, clinically actionable result; diagnosing not just faster but smarter. Furthermore, PCR based detection can enable screening of high-risk patients and help in clinical intervention at the early stages of the infection improving the treatment outcomes.
