HIV-1 & HIV-2: From Diagnosis to Vi...

HIV is no longer the death sentence it once was. Effective treatment lets people live long, healthy lives and stops onward transmission. But every step of that success depends on the laboratory: finding the virus, identifying which type it is, and proving that treatment is keeping it suppressed. Real-time PCR sits at the centre of all three.
UNAIDS released its latest global estimates in July 2026, ahead of the 26th International AIDS Conference. They show remarkable progress, and a response that remains dangerously fragile. In 2025:
· 41.0 million people were living with HIV worldwide, including 1.3 million children under 15.
· 1.2 million people acquired HIV, 42% fewer than in 2010, yet new infections rose in three regions and 21 countries.
· 570,000 people died of AIDS-related illnesses.
· 32.1 million people were on antiretroviral therapy (ART), leaving about 9 million without treatment.
· Around 5 million people living with HIV did not know their status.
· Only 74% of all people living with HIV were virally suppressed.
International resources for the HIV response fell 18% in 2025, the steepest drop in almost two decades. UNAIDS warns that without urgent action, more than 3 million additional people could acquire HIV by 2030.
The gaps that remain are testing gaps: people who have never been diagnosed, and people on treatment whose viral load is never measured.
Antigen/antibody immunoassays are the backbone of HIV screening. But they detect the body's response to the virus, not the virus itself. Results can be indeterminate, antibodies to HIV-1 and HIV-2 can cross-react, and an antibody test cannot tell a clinician whether treatment is working.
Real-time PCR detects viral RNA directly. It answers the questions that guide care:
HIV-1 drives the global pandemic. HIV-2 is endemic in West Africa and found wherever people travel and migrate, and dual infections occur. The distinction matters at the very start of care: HIV-2 is naturally resistant to the entire NNRTI drug class and responds poorly to several protease inhibitors (Shah et al., 2023). A regimen built for HIV-1 can fail against HIV-2.
TRUPCR® HIV 1/2 Detection Kit detects and differentiates both types in a single tube. Three reactions run in parallel: HIV-1 in the FAM channel, HIV-2 in the HEX/VIC channel, and an endogenous internal control in the ROX channel that checks extraction and amplification in every sample.
The assay works on serum or EDTA plasma and detects down to 500 IU/mL. One run tells the laboratory whether HIV RNA is present and which type it is.
WHO recommends viral load testing as the preferred way to monitor people on ART. Suppression is not only good for the patient. The HPTN 052 trial showed early treatment cut HIV transmission by 93%, with no linked transmissions while the virus was stably suppressed (Cohen et al., 2016). The PARTNER study then found zero transmissions in couples where the partner with HIV had a suppressed viral load (Rodger et al., 2019). This is the evidence behind Undetectable = Untransmittable.
Regular viral load results confirm that a regimen is working, flag adherence problems or emerging resistance early, and tell clinicians when to switch therapy.
HIV-1 mutates rapidly, and a mismatch at a single primer or probe site can make a single-target assay under-quantify the virus. TRUPCR® HIV Viral Load Kit amplifies two conserved regions so that one target can carry the signal if the other is affected:
· Dual targets in the HIV-1 gag and pol genes
· Quantification from serum or EDTA plasma, with sensitivity of 200 IU/mL
· Calibrated against WHO international reference materials
· Hot-start chemistry to minimise non-specific amplification
· Endogenous internal control in every reaction
· Compatible with a range of open real-time PCR instruments
Together, the two kits cover the full care pathway, from first diagnosis to long-term treatment monitoring.
| KIT | TARGET | LIMIT OF DETECTION | HIV TYPES |
|---|---|---|---|
| TRUPCR® HIV 1/2 Detection Kit | Conserved regions of HIV-1 and HIV-2 | 500 IU/mL | HIV-1 and HIV-2 |
| TRUPCR® HIV Viral Load Kit | HIV-1 gag + pol genes | 200 IU/mL | HIV-1 |
The world has the tools to end AIDS as a public health threat, but only if people are diagnosed, treated with the right drugs, and monitored until the virus is suppressed. With international funding shrinking, laboratories need testing that is accurate, affordable and runs on the instruments they already own.
