Cytomegalovirus (CMV) rarely makes headlines the way flu or COVID-19 do, yet it is one of the most widespread viruses in the human population. For most healthy people, CMV infection passes unnoticed or causes only a mild, flu-like illness before the virus settles into a lifelong, dormant state in the body. The story changes sharply, however, in pregnant women, newborns, transplant recipients and anyone with a weakened immune system—in these groups, CMV can reactivate or cause primary infection with serious, sometimes lasting, consequences.
Global studies estimate that roughly 8 in 10 people worldwide carry CMV antibodies, with seroprevalence higher still in developing regions and among women of childbearing age. The burden is especially heavy across Asia: CMV seroprevalence among women of reproductive age in India is reported at over 90%, and antenatal screening studies from Indian tertiary care centres have found CMV IgG positivity ranging from roughly 60% to more than 90% depending on the population studied. In the United States, by way of comparison, around one in three children is already infected by age five. Because the virus is so common and so often silent, the real clinical question is rarely “does CMV exist here?” but “is it active, and in whom does that activity matter?”
CMV becomes a genuine threat in a few well-defined situations:
· Pregnancy: Primary CMV infection during pregnancy can be transmitted to the fetus and is a leading infectious cause of congenital hearing loss and neurodevelopmental problems.
· Transplant recipients: In solid organ and stem cell transplant patients, CMV reactivation is a major cause of post-transplant complications, making routine monitoring an established part of post-transplant care.
· Immunocompromised patients: Individuals living with HIV/AIDS or on immunosuppressive therapy are at higher risk of CMV disease affecting the eyes, lungs, gut and other organs.
In each of these settings, the clinical need isn't just to know whether a patient has ever been exposed to CMV, but to measure how much active virus is circulating right now—information that guides decisions on starting, adjusting or stopping antiviral treatment.
Serology (antibody testing) can tell you whether someone has been exposed to CMV at some point, but it cannot reliably distinguish an old, resolved infection from an active one, and antibody responses can be unreliable in immunocompromised patients whose immune systems may not mount a normal response.
Real-time PCR closes this gap. By directly detecting and quantifying CMV DNA in plasma, it gives a current, numerical readout of viral load—the same measurement clinicians rely on to decide when antiviral therapy is needed and whether it is working. A built-in internal control in a well-designed assay also confirms that the sample itself was adequate and processed correctly, reducing the risk of a false negative going unnoticed.
