The Quiet Epidemic: Why smarter diagnosis is needed for STIs
Every day, more than a million people have sexually transmitted infections, but most of them don't realize it, don't get treatment, and pass it on without saying anything
A Quiet Crisis: The Huge Problem of Sexually Transmitted Infections
Sexually transmitted infections (STIs) are one of the most common but least talked about public health problems in the world. The World Health Organization's Global STD Report says that there were approximately 374 million new cases of STIs in 2020. The real global burden is much higher when you include viral agents like the herpes simplex virus (HSV-1 and 2) and bacterial co-infectors like Mycoplasma genitalium, Gardnerella vaginalis and Ureaplasma urealyticum/parvum.
The clinical effects are serious and wide ranging. Chlamydia trachomatis has around 89 million cases each year and is one of the main causes of pelvic inflammatory disease, ectopic pregnancy and tubal infertility. Neisseria gonorrhoeae is quickly becoming resistant to antibiotics, making early detection an urgent public health issue. There is a link between Trichomonas vaginalis and a higher risk of getting HIV and bad pregnancy outcomes. More than 90% of primary HSV infections are completely asymptomatic which means that millions of people carry and spread the virus without knowing it. Co-infections with more than one pathogen at the same time are common and make clinical management even harder. Missing even one pathogen can make the situation worse.
Prevention is a Partnership: Protecting Yourself and Your Community
To stop STIs, everyone needs to take responsibility and work together. Using barrier methods consistently during sexual contact is still the best way to stop the spread of both bacterial and viral STIs. Limiting concurrent partnerships and practicing open communication about sexual health history are equally important measures, particularly because the majority of STIs produce no symptoms in their early stages, making reliance on symptoms alone dangerously inadequate.
Routine screening at least once a year for sexually active adults and more often for those with multiple partners is key to stopping the spread of STIs without anyone knowing. Notifying a partner right away when an infection is found, testing each other before starting a new relationship and not treating yourself are all practical steps that can have a real impact on the spread of disease. At the population level, public health education and the destigmatization of STI testing are the foundational elements that ensure the sustainability of individual preventive behaviours.
Detecting the Unseen: How Molecular Testing Changes Everything
Culture and microscopy are two common ways to diagnose STDs but they don't always work well when it comes to sensitivity and turnaround time. A lot of infections go undetected because the pathogen load is low or the person doesn't show any symptoms. Molecular diagnostics is a game changer in this area. Molecular methods, especially real-time PCR (Polymerase Chain Reaction), are the most sensitive and specific as they can find genetic material that is specific to a pathogen. These methods make it possible to accurately identify infections even when the microbe load is very low.
Syndromic panels are a huge step forward in this area as they let you find more than one pathogen in a single sample at the same time. This is especially useful for STDs because co-infections are common and symptoms often overlap. Early and thorough detection not only helps patients get better but it also stops the spread of the disease.
Molecular diagnostics are the best way to diagnose STDs today because they have a quick turnaround time, fewer false negatives and can find more than one pathogen in a single workflow.
TRUPCR® STD Panel Kit: Eight Pathogens, One Simple Answer
The TRUPCR® STD Panel Kit from 3B BlackBio Dx Ltd. is a CE-IVD-certified multiplex real-time PCR test that can find nine STI pathogens—C. trachomatis, N. gonorrhoeae, M. genitalium, HSV-1 & 2, G. vaginalis, Ureaplasma urealyticum/parvum and T. vaginalis, at the same time in urine, rectal and genital swab samples. An integrated endogenous internal control checks the quality of the extraction and shows if PCR is being blocked in each reaction.
Clinical validation on 168 blinded specimens against a CE-IVD certified reference method demonstrated outstanding real-world performance.
Ø 97.47% Sensitivity
Ø 98.88% Specificity
Ø 98.72% PPV
Ø 97.78% NPV
Conclusion
The need for precise, quick and thorough diagnostic solutions grows as the prevalence of sexually transmitted illnesses keeps rising. In order to overcome this difficulty, molecular diagnostics, in particular, multiplex PCR-based syndromic panels, represent a major advancement. In addition to improving clinical results, these technologies are essential for breaking the chain of transmission and protecting public health by facilitating the early detection and accurate identification of several infections.
